For 10 months I have been itching everywhere except my face.In February my ALP was elevated In April it was normal .Now I am at the 3rd Doctor a Hepatologist and my mrcp was normal but my AST ALT GGT are high .Also bile salts are high I was put on Urso made me itch worse now on Rofampin still itching terrible. I also have high HGB and a sister with Polycythemea Vera...

It is quite usual to get such kind of pruritus due to high AST, ALT, GGT and bile salts which are indicators of any liver abnormality.
It is basically due to high level of bile acids and salts along with its bye products which causes pruritus after coming in blood.
It is quite necessary to reduce the bile alts in blood other wise no medication would be effective in your kind case.
In such kind of cases I prefer to give a tablet containing cholestyramine which is a bile acid sequestrant, which binds bile in the gastrointestinal tract to prevent its reabsorption.
I suggest you to meet a dermatologist and gastroenterologist so that the proper treatment can be taken.
