What Causes Breast Swelling In A Woman Having A Ruptured Saline Breast Implant?

I have 22 old saline breast implants. One of them ruptured almost 2 years ago and I never had it removed. 2 weeks ago I had a car accident and the seat belt was on the side of the ruptured implant. It was very sore to touch for a few days. Next thing I know it s swelled up bigger than my non ruptured one. What could be causing the swelling and how serious is this?
