Noted the history about your 42 year old wife and understood the concerns.
There are multiple problems which may be related.
For instance long term Ibuprofen is sure to cause gastritis and related problems like GERD that is Gastro-esophageal reflux disease. This should be diagnosed by an upper GI Endoscopy and treated with a PPI like
Omeprazole, antacid and
Domperidone if GERD and
esophagitis is present.
Obviously Ibuprofen has to be stopped
Chronic UTI means there is a precipitating cause like you wrote a handing uterus and/or
urinary bladder. There can be stones, strictures, sexually transmitted bacteria as you may be harboring or may be diabetes in either of your or both.
This should be fully diagnosed by Urinary culture and sensitivity,
Ultrasonography, intravenous pyelography or a CT scan and Cysto-urethroscopy.
Antibiotics are prescribed as per the report and have to use protection during sex and you also have to take a course of the same antibiotic for 5 days.
Stress and anxiety adds to any problems like gastritis and GERD and can enhance the symptoms of many a problems.
Get this too treated with medications if required as this is also long standing for 6 months.
