What Causes Chronic Achilles Heel Pain?

I ruptured my right Achillies in 07. I have been able to play soccer and run but of late I have woken up with extreme pain 1.5 inches from the heal. It is that painful that you cannot and i mean cannot even lightly touch it. Touching it gets me closer to God. However after about a week and being on anti-inflamatries it does settle down for about 6 weeks and then happens again. What do you suggest I do? I have already had more x-rays and ultral sounds and they show nothing in particular. My blood count is normal showing no infection of the area. As well as the pin at the bottom of the achillies is not infected either. I am very frustrated as I am only 31yrs old
