Hello
Cold fever and back pain with high ESR in would advise you first omto rule out acute infection like viral bacterial or tuberculosis.

Since you are already diabetic so chances of contracting infection is even higher

I would advise you follow dietary modifications
1. Low calorie diet 1800 calories only
2. Have whole grains Brans, oats and lots of dietary fibers
3. Citrus fruitso and raw or steamed vegetables
4. Exercises as much you can very easily do?

Hope this answers your question
please feel free to ask for more clarification I will gladly answer you
Best wishes
