I can understand your concern for dark red spot on shoulder blade. After analyzing your query it appears that you have got pigmentation after the accumulation of the melanin pigment over the shoulder blade. It is due to the fact that after injury there is more activity of the melanocytes overt the affected injured area which can cause deeper pigmentation which can persist for months to years.
For this kind of dark spot I prefer to give a cream containing kojic acid or arbutin for two times a day. You can discuss further with a dermatologist. Take care.
