I have gone through your query and understand your concern towards your problem. I can understand that your symptoms could be due to depression or anxiety due to chronic back pain or because of your illness.
I can say that it is not merely due to the illness. This is because how you are responding to it is based on your capability to tolerate the effects of illness.
There is nothing to worry. Be confident and hopeful. You can start antidepressants like Escitalopram to get rid of these symptoms. And the confusion is just because you're worried about your illness. Otherwise, it is not common at your age.
Hope this information is helpful.
Take care
