What Causes Fever, Shivering, Reduced ESR With Some Fault Shown In X Ray?

Hi A 60 years female pt with fever and shevering and ESR:104 on same day but after 1 day reduced to 84 .chest x ray was shows some fault,some back pain chronic complaint.now no fever and she is the case of pacemaker .what mostly diagnose can be? Thanks

Increase in ESR shows infection as there is some fault in x-ray, she may be having acute or chronic lung infection
