What Causes Foreskin Tear Post An Intercourse?

Im having a difficult time with the foreskin on my penis. Once it heals, and everything seems fine, I have sex and it tears again. Im a very healthy person, without a lot of health issues, but this has been a real Pain. Do you have any suggestions. I do use Lube in attempt to avoid and aggravate the tissue, but still I have problems with tears. Can you suggest any other solutions.
