What Causes Frequent Blacking Out With A History Of Anemia?

I am 13 and have had anemia since I was 4 years old and lately if I stand up I just collapse to the ground because my vision blacks out and I get so lightheaded and can't catch my breath I lay there for a few seconds then get up and feel in shock. Why does this happen?
