His symptoms are indicative of injury, or inflammation on the area hit. It can also be prostate issues. After trauma urethral spasms, trauma to the urethra may also cause such sensations. Get him clinically examined for internal tear, trauma, injury, etc., tests and investigations like clinical examination, rectal exam, blood at meatus, perineal or penile hematoma, voiding dysfunction, urinalysis, RUG, retrograde urethrogram under fluoroscopic guidance to assess damage, etc. Get him tested at the earliest before symptoms worsen.
