Hi welcome to HCM...

Noted you are mentioning chest pain and breathlessness since one year...
Which cardiac investigation you have done???
First rule out cardiac cause by.....
Measure your blood pressure ....
ECG and 2d echo done....

If no cardiac cause found then respiratory auscultation done....with physical examination.
History taken about cough , fever etc...
Then according to need investigation done like chest x ray and spirometry...
Pulmonary function test will able to rule out restrictive or obstructive cause ...

According to cause treatment given ..like bronchodilator for COPD like cause...

Consult physician with keeping this in mind.

Take care.
