What Causes Hypoechoic Mass In Posterior Myometrium?

uterus is globularly enlarged measuring 117x86x85mm with a well defined hypoechoic mass in the posterior fundal wall measuring 94x65x91mm. Also noted is an ovoid hypoechoic mass within the endometrial canal measuring 15x23x20 mm. Do I need surgery? Can I still get pregnant? TTC for 2 years now
