What Causes Intense Back Pain After A Mild Injury?

My dad is 103 yrs old and in a nursing home. Two days ago he was on the potty and stood up by himself. He stumbled a little and hit his side/back on the pointed end of the sink counter. He did not hurt right away but now is in excruciating pain when he moves at all or even takes a deep breath. An exray was done on site, and the results just came back negative. What could the other possibilities be? Could he be having internal bleeding? Or would they have seen that on the ex-ray? He is mentally as sharp as a tack. He is blind.

old age patients usually have weakened bones because of osteoporosis. but since the xray came negative another possibility can be nerve root compression . he should be given vitamin B12 capsule which acts a nerve protector. full rest along with non steroidal anti inflammatory drugs like diclofenac can be given. back support belt and warm compresses as supplement .
