What Causes Irregular Menstrual Bleeding After Removing Mirena IUD?

I had a Mirena IUD removed in May and I have been having irregular menstrual bleeding since October. My husband and I are trying for a baby. I found out last week that I was 3 weeks pregnant, and then miscarried. I have been bleeding since Wednesday, very heavily. Since Sunday, I started bleeding and have have to change a Super tampon every 30 minutes to 1 hour. I've not been able to sleep more than 3 hours, since I will wake up dripping blood. My doctor prescribed Sprintec to regulate my cycles and I am 3 weeks into the prescription. I informed doctor today of the extreme bleeding, and was told that unless I am anemic there is nothing she can do. She said that if I am anemic, then she will do surgery, but didn't give any more information than that. What are my options?

Hallow Dear,

You were pregnant and then suddenly you have started bleeding heavily. Your tampon count does indicate that you are bleeding very heavily. This seems to be a symptom of abortion. When you say 3 weeks' pregnancy, I presume 3 weeks after missing period. Since you are bleeding so heavily for about 3 weeks or more without any relief with Sprintec (which are norgestimate and ethinyl estradiol tablets) the chances are that you have incomplete abortion. Such bleeding is never controlled with spintec like oral birth control pills.

I would advise you to get ultrasonography done which can tell you about the status of your abortion. If diagnosis of incomplete abortion is confirmed, you will have to undergo surgical evacuation of uterus. Till your uterus contains some products of conception, may be in part, your bleeding will not stop.

If you are bleeding for 3 weeks due to incomplete abortion, the chances of infection are there. Infection will start foul smelling vaginal discharge which will have to be treated by antibiotics.

Due to 3 weeks' bleeding, you must have lost lot of blood. Please start iron tablets and protein to recoup the loss of blood. Get your blood examination done to find out the haemoglobin levels. Though your doctor does not agree with your anaemia, let us confirm it by blood examination.
