What Causes Irregular Periods Along With Lower Pelvic Pain And Stomach Cramps?

So my period has been irregular the past 5 months, anywhere from 1hour of heavy bleeding to light pink spotting for 3hours to absolutely nothing. I am also bloated, having pain in my ovaries and uterus, tired, and have cramps. The urine pregnancy test came back negative, but I am going to get it checked with a blood test just to be sure. My period has never been irregular (I have been on birth control for 7 years), so I m not sure what s going on. I do have cysts on my ovaries (both of them) but the dr said they aren t the cause. But I need a second opinion please..

The possibilities for the irregular bleeding in the current scenario are- weight changes, co-existing infections, use of medications that may interfere with the hormone milieu, stress and the last, polycystic ovaries. You will need a complete hormone profile to detect any deficiency. A complete blood picture, coagulation profile since you are using hormone contraceptives over years, urinalysis are also needed to exclude any pelvic and systemic infection. A clinical evaluation should be done and a sonogram of the pelvis, preferably through the trans-vaginal route, should also be done if not done already. This clarifies the picture and further management can be planned accordingly. Hope this helps.
