Noted the history related to your son - nine year old son scratches his
scrotum, below his balls consistently when he is at home. He doesn't do it when we are out and about? But it seems to be driving him crazy...tried cantesten powder etc..No red rash just said it really itchy mum.
The most probable diagnosis I can think of in such a small child having itching scrotum are:

Atopic dermatitis of the skin of the scrotum, although this is more common on the legs but can appear anywhere on the body. Possible on the scrotal skin due to its rugged appearance.
- Retention of hte soaps also can start the dermatitis.

Allergy to the clothes he is wearing.
- Playground allergens so well known in this age group.
I would advise you the following:
-Stop using all the
ointments or local medication, soaps and such things.
- To get pure cotton underwear and to change them at least twice a day and particularly after a visit to playground.
- Have one consultation with a
Dermatologist for clinical diagnosis and further tests and treatment as per the clinical diagnosis.
