Hi...you are right...if its only single joint it is highly unlikely to be a Rheumatic fever. More so we should have to consider the possibility of - 1. Reactive arthritis 2. Septic arthritis 3. Pauci articular rheumatoid arthritis (this is different form rheumatic fever). I suggest you seek your paediatrician's help with these tips. Hope my answer was helpful for you.
