Welcome to healthcare magic
I am Dr Fahim and I will help you with your query

I have through your question. You have not mentioned site of this lump? From his reluctance to see a Doctor and altered bowel habits I presumed it to be in peri-anal region.
It needs proper examination and seems to be anal fistula, in which there is pus discharge, a small lump can be there. He needs to see a general surgeon, who will perform an examination. He needs not to be sacred and shy, If it is there this problem for one year, it needs his attention and prompt treatment.

I hope my answer will help, Do rate, if you like it.
Regards
