What Causes Nose Bleeds When Bending Over?

Frequent Nosebleeds when bending over – hard to stop Hi, 58yr old male, 5’11” and 275lb’s. I my blood pressure runs between 120/75 - 125/80 most of the time, but I have seen it in the neighborhood of 140/95 on occasion. Cholesterol is slightly elevated. Recent heart stress test (chemical induced) came out fine. My question is concerning nose bleeds. Throughout my life I never had nose bleeds unless there was trauma. However, over the past 7-8 years I have been getting increasing nose bleeds from just bending over (head at knee level or below) such as to pick something up, tie a shoe. If I strain while bent over I will most often get a nose bleed, but also it occurs without any strenuous activity also. I can tell when it is going to happen because I feel a sudden kind of blotting feeling or a pressure around my eyes and sinus area.Once the bleeding starts it flows pretty heavy with say a drop about every second or two. Getting it to stop sometimes takes 20-40 minutes or sometimes longer. I normally sit down and tightly pack my nose with tissue paper. If it does not soak through I leave it in for at least 15-20 minutes. Normally when I withdraw the tissue there are large clots of deep dark red blood. Sometimes it will start bleeding again and sometimes not. If it bleeds again I repack it. Most of the time after such an event, say about 20-30 minutes after I have controlled the bleeding, I cough up a rather large (more than a table spoon) clump of blood which is very dark red in color. While in the doctors office on another matter, I’ve briefly mentioned this to my doctor but it did not seem to raise concern with her. Is this something to be overly concerned about?

I have read your complaints and understood your concerns. Nose is richly supplied by blood vessels. So slight trauma/injury or dryness cause bleeding. As you mentioned your blood pressure is normal.
If I were your doctor, I would immedietely advice an endoscopy to know the source of bleeding. Blood counts have to be tested. The treatment would be according to the pathology. Oxymetazoline nasal drops control acute episodes. Very anterior bleeding can be blocked by chemical or electro cautery.
Nasal irrigation salines help in prevention of nasal dryness.
