Hi,I am Dr. Gopal Goel (Orthopaedic Surgeon). I will be looking into your question and guiding you through the process. Please write your question below.
My left leg has numbness along the perineal nerve from thigh to top of foot and tall toes. I experience tingling at ankle when flexed and along sole of foot when bending at waist. I have complete feeling in the sole of my foot. The big toe on this foot is weak, bent downward - I’m unable to flex it upward and it alone drags when I walk (not a foot drop but a big toe drop). ENG/NCVs of left leg show weakness. Cervical MRI showed 3 mild to moderate spinal chord impingement and bone spurs but not indicative of causing left foot / big toe symptoms. Symptoms occurred following a fall resulting in hairline fractures of left tibia and fibula (near knee proximity). Lumbar MRI scheduled. Could L5 - S1 disc involvement cause only big toe drop? I have no lumbar pain or any residual pain from fractures (now 4 weeks healed). What else could cause the single digit / big toe drop and thigh numbness down through top of ankle and foot/toes?
Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.
Inability to raise big toe in your case considering clinical history can be:
1. L4-L5 disc herniation with L4 root compression.
2. Involvement of flexor tendon in callus at fracture site.
MRI of the lumbo sacral spine is required to confirm diagnosis. Tingling and numbness from ankle to thigh is also sign of nerve root compression in lower lumbar spine. So we should wait for MRI report to confirm diagnosis. Meanwhile, start tab. Pregalin to decrease tingling in left lower limb.
Regards.
