What Causes Pain Above The First Metatarsal?

Hello, I am an 18 year old preprofessional ballet dancer and i dance an average of 5-6 hours a day. Nearly two years ago i developed pain in the top of my foot. I was in the area right above my arch/ first meditarsal. I had a sharp pinching pain when i would put weight on it, walk, jump and dance. I saw an orthopedic doctor who specializes in foot and ankle problems. An x-ray showed nothing and nothing was seen on an mri. He put me in a walking boot for 4 weeks and then prescribed some physical therapy. I was also told to rest however I was unable to rest due to a very important performance. I was told i either sprained or strained a ligament on the top of my foot. My PT instructed me to wear the boot for another few months while i was at school because my highchool was very large which required a lot of walking through the day. The pain is now returning. I talked to some of my teachers and they feel as though i am working toward a stress fracture. I am experiencing pain in some new areas this time. It has spread to the second and almost the third meditarsal from where my foot/ankle meets my lower leg to the knuckles just before my toes. I dont have any bruising. I am also experiencing pain right where my foot meets my ankle. Do you think this is a stress fracture? Am I working towards a stress fracture? Do you think I will need to wear any type of cast or boot? P.S. I also have significant scoliosis. Its an s curve and the two curves are close to the same degrees. I dont know if that will effect anything...it seems that any type of pain i get somehow traces back to my scoliosis. Thank you.
