I have gone through your question and understood your concerns. Its normal to have discrepancy in testicles size, usually the right one is a bit bigger and left one is lower than right. If one is bigger more than 50% of the other testis, increasing in size with time, hard in consistency and without pain then you needs urgent evaluation to rule out malignancy. The first investigation is usually ultrasonography of the scrotum. It will give us an idea about composition of the testis, any abnormality will be visualized and reported. To further rule out malignancy, alpha fetoprotein and Serum LDH is advised. However, If the increase in size is associated with pain and swelling then its most likely an infective process, which is easily treatable with empirical antibiotics therapy. You need to see a general surgeon or an urologist for evaluation and subsequent treatment.
