What Causes Pain During Defecation, Constipation And Stomach Growling While On Nexium?

I am 78 and have been taking Nexium for at least 20+ years. I have a very acid body. Before that, I chewed up about 6 Tums a day and 3-4 at night. I have had low blood pressure all my life and still do. I never get sick, like colds, flu and ordinary stuff most people get. Also, I do not get any preventative shots for anything ever. I am strong and healthy a do lots of outside work around my six acres, but lately I ve been having digestive problems. Large, solid BMs that hurt and then nothing for 3-4 days. Stomach gets hard, growels, have lots of gas and burp a lot. Also feels like have a sharp strike (like lightening) inside my abdomen on and off. What is happening to me?

Hi.

Nice to know that you are 78 years old and till recently you had not problems of any sort, taking Nexium and tums for 20 years but got the gastrointestinal problems now.
The pain in abdomen, growling, large solid bowel movements with pain and so on can be due to the following reasons:
More absorption of water from large bowel as you are working in your 6 acres.
Dietary changes.
Something in colon or rectum that needs investigations.
Hence I would advise you the following in such a situation:
Consult your Doctor for clinical evaluation and examination; per-rectal palpation to see for any mass and ano-proctoscopy to see for any piles or polyp, mass.
X-ray of abdomen to see for any bowel distension, fecoliths and if present to go for stool softeners and laxative to completely empty the bowel.
If this does not work then you need to undergo colonoscopy and biopsy, CT scan of abdomen.
Blood tests CBC and all along with tumor markers.
Tests of stool.
All these will lead to a proper diagnosis and rule out anything serious like cancer of colon.
Take a PPI and Domperidone for the burps to settle. if these do not help then an upper GI Endoscopy may help.
Go for the management as per the findings.
