General Surgeon 's  Response
Hi,
Varicocele is a common problem and does not usually cause severe pain. What it can cause is a dragging sensation or heaviness in the scrotum after standing for long periods. People dont usually require surgery for it unless it is leading to low sperm counts or is Grade III leading to pain.
Both testicles are not equal in anyone. So dont worry about that. It it normal to have unequal testicles.
You dont need to get operated. Just wear tight undergarment so that the scrotum does not hang loose from the body and avoid prolonged standing for a few months.
Take care,
Dr Rishi, New Delhi, India.
