What Causes Pain While Suffering From Aerosinusitis?

Hello, I m 99.9% sure I experienced aerosinusitis or barosinusitus on a flight back from New Orleans on Monday. This was the second time it has occurred and the experience fits all of the symptoms & information I can find. Absolutely the most excruciating & debilitating pain I’ve ever experienced! I don t have a cold or nasal congestion, drainage, coughing...nothing to indicate my sinuses are compromised so I can only assume it was caused by the pressure change. The issue is the persistent headache since Monday- it ebbs and flows with severity but it s focused over the same area that felt like it was going to implode on the plane (right eye, eyebrow, inside of nose, over the brow and now radiating into the right temple and around the top and side of my head) How long is this headache going to last? I m at 5 days of it and today is by far the worst! I ve taken Tylenol, ibuprofen and nothing touches it. Should I be concerned? Should I see a physician? I just need some relief... Thank you, Erin

Hello dear,
After going through your history, I believe you have some genuine concern and I will try to help you out of this situation.

My answer follows your diagnosis of Barosinusitis.

Our nose is surrounded by sinuses. Sinuses are empty cave like spaces in the facial bones with their mouth opening into the nose. These opening are very small and lined by epithelium same as that of nose. In a normal situation when we breathe, air enters the sinuses and ventilate them. Also there is constant production of fluids inside the sinuses, which drain out through the small opening (mouth) to the nose.

In Barosinusitis there pressure difference of air between the nasal cavity and air inside the sinus. This usually occurs when the lining epithelium of mouth of sinus is affected by some cold or infection and is in a swelled condition closing the mouth of the sinus. So with the flight taking up or coming down, the pressure of air inside the nose varies, but that inside the siunus can not as the mouth of sinus is closed. This brings the condition called barosinusitis and typical symptoms as you described.

In my clinic, my 1st aim is to opening up the mouth of sinus with decongestant drops like Xylometazoline/Oxymetazoline. After that some oral decongestants along with steroids and/or antibiotics can be prescribed depending upon the condition of the nose, which must be examined first with an endoscope.

I advise you o take the nasal drop for atleast 2 days and visit a good ENT specialist if required.
