What Causes Painful Bowel Movements After Delivery?

I had my daughter 7 weeks ago and had a 4th degree tearing. Having a bowel movement have been painful since than but using stool softner help but now it's rare to have a bowel movement and when i do it really hurts and there's also blood. In my 6 week pp I was told I had a rash (gardnerella) and a piece of tissue in the middle of the vagina.I was wondering if that's the cause of it or if there's another reason for the painful bowel movements.thank you

I have gone through your query and can understand your concerns..

As per your complain painful bowel movement after delivery can be due to a common condition that occurs are anal fissure, hemorrhoids and it can cause pain and blood during bowel movements..
Perineal tear can also be a cause that you are already aware of..
Gardenella is generally causative factor for Bacterial Vaginosis and is not seen to cause the symptoms of painful bowels and it can cause vaginal discharge, at times bad odour from vagina but bowel difficulties are not due to it.
I would suggest you to consult a Proctologist and get evaluated and he can do a clinical examination and if needed an anoscope can be placed in the anus to see the inside of the anal canal to rule out the cause..
In case if it is due to fissure you can be advised to apply ointment containing Calcium Channel Blocker like Diltizem as it will improve blood flow in the area by dilating the blood vessels and fasten the healing process..
Sitz bath that involves sitting in a tub of warm water 3 to 4 times a day can help.
Taking stool softeners and high fibre diet can help in smoothing the stools and ease in evacuation..
Avoid spicy food as it can also cause burning sensation..
In case of no improvement surgical correction can be advised..

In case of hemorrhoids or piles steroid ointments and oral anti-inflammatory painkillers can help in relieving pain and inflammation..
Other treatment modalities that can help includes sclerozing agent injection, rubber band ligation, cauterization and surgery as a last resort if nothing else works..
