General Surgeon 's  Response
Hi.
Pulling tight and extremely painful testis in an young boy of 17 needs urgent ultrasonography and color doppler examination to confirm testicular torsion so that urgent and timely surgery can save his testicle.
If the torsion is not present, the other diagnosis will be clear like cyst or so and helps to get the plan for further management.
Rush him to ER.
Give scrotal support and do not give him anything orally in an anticipation of likely surgery that he may need.
