What Causes Perineum Bleeding?

I am a 34 yo male 6'1, 250 lbs. I have perineum bleeding that does not seem to want to stop. Been to the doctor several times over the last few months about this cause I have had an abscess in that area in the past. Doctor prescribed an antibiotic months ago, went for a visit after and he suggested just keep area clean and take a warm bath or warm compress regularly. The are is still not healed. Just returned from a business trip where I had to fly and the bleeding has gone from a slight drop to a stream when i go to the bathroom for a bowl movement. What is the cause of this and what should i do to heal it.

I really appreciate your concern, if this was the history of abscess of perineum region then it might not be eradicated completely so the lesion or the infection might be occurring on and often, it seems to be surgical condition that may needed complete draining of area, abscess pockets needs to be opened and drained completely, pus needs to be sent for culture sensitivity, (If it is the pus formed there) else the debris needs to be sent for lab examination hope this helps.
