What Causes Persistent Headache Followed By Dizziness & Tingling In The Eyes And Face?

21 yr old healthy girl with 1 month onset of constant headache and dizziness, eyes and face feel tingly. MRI neg, blood work including thyroid WNL, OBGYN took her off BC 2 weeks ago - no improvement. Saw neurologist and dx of migraines - gave maxalt and an anti-anxiety med. Nothing has helped. Headaches are dull, pressure, aching behind both eyes and frontal lobe. Also, opthamologic exam completely normal. No nausea and sleeping, eating well. Any thoughts?

Regarding your concern, I would like to explain that her symptoms seem to be related to a tension type headache or a new daily headache.

The facts that she has no nausea, and all her tests (including the MRI) have resulted normal are all arguments in favor of these diagnosis. The diagnosis of sinusitis is excluded with the MRI findings.

So I would recommend to start on an antidepressant like amytriptiline or nortriptiline. If she has not good tolerance to these tricyclic antidepressants, I would advise paroxetine or fluoxetine.
