What Causes Recurring UTI?

What would be your diagnosis for a person who has consistent recurrences of UTI infections. Symptoms include passing of bright red blood, severe low back pain, bloating and cramps. Has been on-going for 2 years. Have been to the Dr., and urologist a number of times and continue to get the same answer, There is nothing wrong. Have taken many antibiotics. Symptoms will clear up for a time, but comes back. At this time I am having another UTI, on antibiotics, but I don t think I m responding to the antibiotics. Just left another Dr. s appointment and once again told they can find nothing wrong. Scared and extremely frustrated! Can you advise.

I read your question carefully, I understand your concern and I'm sorry that you are in this situation.

Since you didn't write any information about your age, sex and other chronic diseases that you might have which would have been helpful in being more specific about your situation, I will try to give you detailed advises in a general way. I will be more predisposed to write about this condition in women, since it is more probable. But, feel free to write again with additional information.

Recurrent urinary tract infections can be a consequence of several factors. For example:
- Stones in the kidney or bladder
- Diabetes
- Sexual intercourse where bacteria might enter the urethra
- Malformation or abnormal urinary tract shape
- If you are a women in menopause, changes in estrogen levels also might predispose for recurrent urinary tract infections

In this cases I would recommend to my patients:
- Urine culture and urinalysis
- Cystoscopy
- Ultrasound of the kidneys and bladder
- CT scan of the urinary system

Regarding alleviating the symptoms you might consider some lifestyle changes initially:
- Drinking a lot of fluids
- Consuming cranberry products
- Washing daily the skin around anus and urethra using a mild soap and water
- Emptying the bladder as soon as possible after intercourse
- Don't use deodorant sprays in genital area

If the above studies and advises don't help you and the cause remains unknown, you may talk with your Urologist about some helpful options like:
- Low dose antibiotic for a long period (1-2 years)
- Taking an antibiotic after intercourse
- In women in menopause, vaginal estrogen treatment (if you don't take oral estrogen)
