Sometimes I get this really REALLY bad pain in my lower abdomen like halfway between my genitials and rectum and seriously the only way to get it to stop is to ejaculate then it slowly disappears over 15 min or so... Sometimes it’s so crippling I can’t move... what can cause this? I had a doctor go in through rectum 3 years ago for it but he didn’t see anything.

It looks like Prostatitis, get CBC, Urinalysis, imaging tests done. You need to be examined and evaluated (to rule out anal cancer & painful bladder syndrome), visit ER. It can be treated with antibiotics(tabs or IV as required), Alpha blockers and pain (NSAIDS) can be controlled with medications but sometimes immediate hospitalization is needed.
Avoid any addictions, have protected sex, keep yourself well hydrated, avoid cycling, drink caffeine free beverage & take sitz bath.
