What Causes Severe Strain While Urinating?

I was told in october of 2009 by my VA urologist(I have been through 4 of them since) that I needed Urinary Diversion Surgery. He told me that he would NOT reccomend me for it and sent me home to live in pain without it. He also told me that there are NO SURGEONS at the White River Junction VT VA that I get my care at. I have NO medicaid, it was cut. My PCP at the VA will NOT reffer me to another doctor in the VA or elsewher for care and the surgery that I so DESPERATELY need. I am in GREAT pain 24/7, and cant urinate. I have been severely straining for 25 years. The VA originally performed a surgery on me in november of 2001 in Batavia, NY. I had a TURP. They botched the surgery, and I feel there is surgical scar tissue in my prostate, bladder or urethera. I cannot confirm this, however, because I cat get an MRI. I NEED SURGERY IMEADIATELY! HOW DO I GET IT?

As per my surgical experience, the symptoms narrated suggest the possible underlying narrowing of the urethra after TURP surgery.

In my clinic, I suggest the patient undergo evaluation with ascending urethrogram report for best judgment and further management guidelines.
