What Causes Sharp Pain In Arms And Loss Of Sensation In Fingers?

I recently fell 4 days ago off a step ladder onto my left shoulder-back. I am in above average pain which shoots through my arm into my hand, and have lost feeling in my pinky and ring finger. I do not have insurance and was wondering if any home care can be done or should I see a doctor. Thank you.
