What Causes Sharp Rectal Pain During Menstruation?

I get a shooting pain in my rectum during my periods. I am scheduled to go to my gynecologist next week. I have been seeing her for 3 years and she never does a rectal exam. If I tell her about this problem, will she want to do one? The thought of that really makes me nervous and I just want to be prepared.
