Noted the history about your husband who has undergone hernia repair surgery 4 years ago and is having pain while ejaculating since then, kind of twisting and pain in abdomen. The most probable causes are: Tightening of the spermatic cord by repair of external ring. Trauma inadvertently to the vas deference. Nerve entrapment syndrome. Rotation/ torsion of the testicle I would advice the following in such a situation. Ultrasonography of the whole abdomen and scrotum with color Doppler studies. This will give the most probable reason and a plan as to how to get the relief from the present complaints.
