Hello Since you are having shortness of breath, weight 230lbs ( over weight ) , smokers, high Blood Pressure , on B P medicine . So understand this thing. Paroxysmal nocturnal dysponea , also known as "CARDIAC ASTHMA ", this condition is characterized by attacks of severe shortness of breath that generally occur at mid night and usually awaken the patient from sleep. The attack is precipitated by stimuli that aggravate previously existing pulmonary congestion( as may be due to smoking in your case), frequently , the total blood volume augmented at the night because of body during recumbency. Bloating , light headeness , dizziness are due to shortness of breath i.e. cardiac asthma . I recommend you get in E K G & ECHO at earliest and consult your "CARDIOLOGIST " to sort this problem. Hope this information will be helpful for you.
