Hi
I am Dr Mittal
I read your query.
I would have ideally preferred if you could upload the picture.
I will try to explain as i see-

I believe there is a possibility that his sugars may not be well controlled.
I am also assuming he may have injured himself in some fashion. The injury may have been small and insignificant.
But with uncontrolled sugars, it may have led to an infection and gangrenous changes.

I would suggest checking his sugars and tlc.
Also better to visit a physician, who can examine him.
Alternatively you can upload the picture.

Also please let us know the details of his treatment history
Best of luck
Dr Mittal
