Urologist 's  Response
Hello
Thanks for query.
The discomfort in left testicle with feeling of worms is mostly due to Varicocele.
You need to consult qualified Urologist for clinical examination and get following basic tests done to confirm the diagnosis,
1) Ultrasound Scanning of Scrotum
2) Color Doppler study of scrotum
3) Semen analysis.

Further treatment will, depend upon results of these tests .Decision regarding surgery will depend upon the sperm count.

Thanks .
Dr.Patil.
