What Causes Vaginal Bleeding For Almost 4 Weeks?

Hi I haven't had my period since Aug 2013, thenlast mth feb 2014 it comes ( so I thought) I usually get it for 2-3 days, not that heavey... Well I've been having vaginal bleeding for almost 4 weeks..using bathroom a lot, n I'd have small blood clots almost every time I'd go .I get most of the blood when I wipe, bright red, sometimes with dark blood as well. This is annoying, but also wonder why this is happening..?
