Over the past two years I have gained 30 lbs. I have copd and am on 3 inhalers; heart problems and thyroid. Meds I'm on are metoprolol, valsartan, isosorbide mononitrite, levoxythyrine and pravastatin. I had a stroke in 2011 and carotid artery surgery in 2015.

Ok, it is either fat or fluid.
First, figure out which and get x rays, examination and thyroid studies. Puffy ankles would imply fluid.
One strong possibility besides fluid (which is also very possible) is that with the COPD, you were TOO thin from overexercise of the lungs and with some medication, you gained muscle (and fat) that you should have had.
