What Could Explain Swelling Behind Ear, Back Pain, Hip Pain And Lightheadedness?

i am 30 year old female, i have been experiencing some issues for two weeks now that seem to be getting worse. my sx are: fluid in ears, swelling behind ear, 4+ times a day i get really lightheaded feel funny and extreme pressure in my forehead, i am extremely shaking during this time and have to put my head down for those sx to stop, and when i pick my head back up they are still there. it take about 5 minuets for it to disapate. my shoulders are killing me, extreme hip pain, and lower back pain. i have aniexty issues and drepression, as well as ceiliac disease, and i need to get my gallbladder out. i see a family doctor, i see a chiropractor. but nothing is fixing my lightheadedness, pressure in forehead and shaking. the meds i take right now are zoloft 150 1 1/2 tab qd, metformins 500 bid, nasonex i spray each nostril bid, and have been on amoxil 250 bid for the past three days. i do smoke 7-10 smokes a day. but today was the worst sx i have had yet. i was at work all morning went home for lunch and i started to hang up some clothes when i turned around, my eyes became unfocused, i got extreme pressure in my forehead, i was shaking like crazzy that my right knee gave out on me it felt like i couldn t bare weight on it, very weak. i put my head on my bed and everything stopped but as soon as i picked up my head it was there again. this time my vision even got very blurry and i lost my balance a few times. i am overweight, i weigh 250lbs and i am 5ft 3inches. what the hell is going on with me.......
