What Could Have Caused The Heart Failure?

My father has recently died from heart failure. He had high cholesterol and hypertension but no other history of heart problems. His post mortem subsequent showed he had cardio megaly with significant enlargement of the muscle of the left ventricle. A chest x-ray taken 6days before he died showed no signs of this apparently. Looking back he did have a lack of energy for the previous couple of months. 3months prior his dr took a full set of bloods and everything was normal. what could have caused this?

you have not mentioned your father's age,it would help, well, cardiomegaly could be due to various reasons but at advanced age it is more likely due to underlying ischaemic heart disease, his heart muscles could have become too weak to pump out reqired amount of blood leading to heart failure.
