What Could Loose Stools With Loss Of Appetite Suggest?

My grandson has lost quite a lot of weight over the last 3 months. He has loss of appetite and quite often brings his meals back. H is stools are smelly and pale yellow. He looks pale and thin. He also does not sleep very well, falls asleep late and awakes very early. Although i seem concerned his parents do not. Have i any reason to be concerned and what advice do you give?

-Give liquids.
