A 40 y/o male presents with severe pain on his left side and back below the ribs. He describes the pain as spreading to the lower abdomen and groin and comes in waves that fluctuate in intensity. He has painful urination and brownish foul smelling urine. He reports a persistent urge to urinate and urinating more often than usual along with nausea and vomiting.
This could be a number of things including a urinary tract infection, pyelonephritis (kidney infection) or possibly a colovesical fistula (a connection between the colon and the bladder). Foul smelling urine usually indicates some sort of infection, and usually needs antibiotics for treatment. I would recommend that you start by seeing a physician and having check your urine (a urinalysis or "UA"). A renal ultrasound or a CT scan can help diagnose pyelonephritis. If you have recurrent brownish foul smelling urine even after treatment, then a colonoscopy would likely be indicated. Either way, I think the best course of action would be to see a physician and have him evaluate your urine as a start.
