What Could The Cause Of Occasional Lethargic Symptoms?

My husband has occasional disturbing symptoms. Lethargic type symptoms where he can appear not quite drunk but as if he has a buzz. He can become very distracted while driving and today he hit (not hard) a sign post. I am getting worried. It started about 3 years ago. He had a heart attack 7 years ago and has a pace maker/defibrillator. Can his meds have something to do with this? The last time he had one of these spells was about 5 weeks ago. They don t last long.
