I suggest a possibility of
leukocytoclastic vasculitis (LCV) Or
small vessel vasculitis.
LCV classically presents with red spots (pupura) which are palpable (raised) over lower legs and feet, commonly. Arms are rarely involved. The lesions may have a burning sensation or may be asymptomatic.
There may be whole gamut of systemic symptoms like fever, malaise,
muscle pain, joint pain, swelling in legs,
tingling and numbness in arms and legs.
These symptoms indicate that the involvement is not just localized to the skin.
Since small vessel involvement may also be happening simultaneously in internal organs like kidney, GI as well as joints etc, therefore systemic involvement should be ruled out by appropriate investigations i.e urine microscopy, stool for occult blood etc.
Skin biopsy would be confirmatory of vasculitis.
Internal organ involvement may have serious consequences.
Treatment of LCV is with a short course of Oral steroids. Most cases resolve within 2 weeks.
There might be a few relapses but ultimate prognosis is good provided there is no internal organ involvement.
