What Do Right Ovarian Endometriotic Cyst And Left Sided Hematosalpinx Indicate?

Hi Dr. Me and my husband attended pre-conception check with my OB/GYN who has asked us to get ultrasound followed by MRI done. Below is what my report says: Procedure: MRI - PELVIS (PLAIN AND CONTRAST STUDY) Finding: Multiplasnar MR images of the pelvis were obtained using T1, T2 weighted STIR sequences. Study supplemented with post gadolinium contrast sequences. The pelvic inlet appears normal, with normal configuration of the iliac wings and iliopsoas muscles. The uterus is anteverted and has normal internal structure. Uterus measures 7.3 x 4.0 x5.1 cm No focal lesion noted. Endometrial cavity measures 8 mm in anteroposterior diameter .No focal lesion noted Anterior junctional zone measures 8 mm Posterior junctional zone measures 8 mm Right ovary measures 42x27 and shows a cyst of size 22x22 mm which is hyperintense on T1W imaging and shows heterogenous hypointensity on T2W imaging with shading, not supressed on T1W fat sat imaging and is suggestive of ENDOMETRIOTIC CYST Left ovary measures 23X10 MM Left adnexa shows elongated multisepatated structure of size 50x33 mm with hyperinense content on T1W T2W imaging and shows wall enhancement on postcontrast .imaging findings suggestive of left HAEMATOSALPINX . The adequately distended urinary bladder appears normal and has a normal wall thickness. There is no apparent lymphadenopathy noted. The femoral heads are normal shape and articulate normally with the acetabula. They have normal bone marrow signal characteristics. Impression: RIGHT OVARIAN ENDOMETRIOTIC CYST LEFT SIDED HAEMATOSALPINX I was asked to try to conceive naturally for 3 months and then come back if not successful. I m anxious to know what will be my way forward in the event I do not conceive naturally. Do I need to try more than 3 months before consulting again? Do I meet a reproductive endocrinologist for a second opinion? Please suggest.

OBGYN 's  Response
Hello,

You should try naturally for 3 months if it's not relieved your consulting doctor will give you good antibiotics coverage followed by laparoscopic endometriotic cyst removal. Then again you can try naturally.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Meena Jethani,
OBGYN
