Hi doctor I am pregnant,26 weeks,27yrs old,my FBS is always between130-160 now.i m taking human actrapid 22 units at morning & at lunch time& 18 units of actrapid,18 units of insulatard at night. Can you suggest a suitable menu to me to follow?is my baby is at risk? This is my second pregnancy.
this is gestational diabetes . well with this you come under the high risk pregnancy group. now that you are already on insulin , it is assumed that your sugar levels were not controlled by diet/ exercise / or even oral hypoglycemics like glibenclamide or metformin. the target range for the premeal and post meal values should be continue exercise , diet and of course insulin once started should not be stopped. you should undergo 2 weekly blood sugar seven point profile to keep a check on your control. you can discuss the same with your obstetrician and maintain a chart accordngly. Babies of GDM mothers are likely to be macrosomic if sugars not controlled and can cause dystocia during delivery . also a risk of hypoglycemia persists after delivery in babies. lastly pregnancy should not be continued beyond 39 weeks as there are unexplained fetal deaths in GDM mothers after 39 weeks as proven in literature. regards
