Hallow Dear,

There are no adnexal masses indicates that neither there is any infection related adhesions nor any mass or tumour to the sides of the uterus. Your right fallopian tube shows prominence which can be due to hydrosalpinx. Hydrosalpinx is a result of tubal block at terminal end due to infection. The small complex cyst in the right ovary also could be due to trapping of the eggs or minor adhesions due to infections. No free fluid is normal.

Considering your fertility, I feel, the hydrosalpinx on the right side may cause problems. Left tube has not been mentioned; however, it is not possible to adjudge the tubal block by ultrasonography. Tubal block on the left side also may be a possibility.

I would advise you to get your Hysterosalpingography done in which they will instil radio-opaque dye in the uterine cavity and the tubes before taking X-ray picture. This will give us information of precise pathology in the right tube, the condition of the left tube and even the uterine cavity also. If both the tubes are blocked, you may have to undergo reconstructive surgery on the tubes.

It is a good option to find out the status of egg release by ovulation monitoring by ultrasonography. Also semen examination would be useful guide.

I hope this helps you.
